CardioLink, is an automated, voice-first AI triage engine designed to act as a high-leverage extension of the clinical workforce. Operating entirely over a standard phone call, CardioLink's conversational AI proactively assesses a patient’s wellbeing, symptom progression, and functional capacity. It securely translates this unstructured conversational data into standardized, actionable clinical insights and routes natively into the clinician’s Electronic Health Record (EHR) inbox. CardioLink integrates remote monitoring data and structured patient-reported inputs to generate longitudinal summaries that highlight meaningful change and provide actionable context for clinicians. Our platform is designed for use in regulated healthcare environments, emphasizing interpretability, auditability, and clinical accountability. CardioLink does not replace clinical judgment or automate care decisions. We support earlier, more confident decision-making in high-risk cardiovascular populations. Initial focus: post-acute heart failure management and readmission reduction.
