The CardioCARE platform is a comprehensive suite of clinical and nutritional services, activities, and analytics that help patients with chronic or complex conditions. CardioCARE's overarching goal is to be preventative and improve patient health through personalized care monitoring and counseling. CardioCARE aims to improve care coordination, patient outcomes, reduce hospital visits, and boost patient engagement by significantly helping Providers and healthcare organizations meet those goals. The care coordination team becomes an extension of the Provider's practice by collaborating together with their patients outside of the office visits to help patients manage and control their complex health needs.
